JRMC’s ACT program is uniquely designed to provide care, education and encouragement for those with permanent heart conditions such as chronic heart failure. The program consists of three components. First is Assess. JRMC will assess the patient’s ability to participate in the program, and provide education about lifestyle modifications, medication management and nutrition. Each participant will receive an individual plan of treatment. Second is Control. Patients will learn to help control their condition through physical fitness, attending exercise classes three times a week at the CHF Outpatient Therapy Center. Third is Thrive. Participants will graduate from the program with the option of joining the Wellness Center with the joining fees waived. A physician referral is required, and the cost is a 20-percent co-pay per patient.
