CMS Requirements for Employees

To: All Jefferson Regional Employees, Providers, Students, Contractors and Volunteers

On November 4, 2021, the Centers for Medicare and Medicaid Services (CMS) issued an emergency regulation that requires COVID-19 vaccination for employees, physicians, students, contractors and volunteers plus any individual who provides care, treatment or other services for Jefferson Regional.
These are the steps and dates which we need your compliance with:
- 1. Provide documentation of your COVID-19 vaccination to Human Resources by November 29, 2021.
or
- 2. If you have not received your COVID-19 vaccination, receive your first dose of the Pfizer or Moderna or the single dose of the Johnson & Johnson vaccine by December 5, 2021 in Employee Health or if done elsewhere, provide documentation to Human Resources.
or
- 3. Complete a medical or religious exemption request form, see attached, and return to Human Resources by November 24, 2021, at 4 p.m.
COVID-19 vaccinations will be available in Employee Health starting Monday, November 15th from 7:30 am – 4 pm, M-F. You can also receive a vaccination at HealthWorks or one of the local pharmacies.
